Social Services and Admissions

Whether you are looking for a short respite stay, rehabilitation to return home, or a longer stay, Heartland Country Village may be the place for you or your loved one.  Admission arrangements may be made by the patient, the family, the hospital discharge planning staff or a physician. 

Our qualified, caring staff will assess your needs and help you and your family determine if our facility is the right place for you.  Our Social Services Department will help ensure that you and your loved one have a smooth transition.  We will assist in the initial adjustment by providing information on Medicare benefits, Medical Assistance eligibility, and Resident Rights. 

While you are here we are always available to discuss your needs.  As needed, we will complete a home assessment and provide referrals to home health agencies during your discharge planning to facilitate your move back home.  Your overall experience at Heartland Country Village is important to us, and we strive to make sure your experience is a positive one.    

We understand the decisions you make regarding rehabilitation and long term care are not easy ones.  We want to help you and will guide you through all of the various stages you and your loved one may encounter.  There are links below that may assist you as you gather information to make an informed decision.
